2. Verify and Review Medications

Ensure the accuracy of the information gathered by verifying it with multiple sources to prevent the oversight of potential drug interactions or the omission of essential medications from the patient's regimen.

Reviewing the list with the patient or caregiver ensures that all medications are accounted for and prescribed correctly.

3. Identify Discrepancies and Clarify Information
